At Living Gracefully PLLC, we are a resource and conduit to build relationships focused on living well. We are care professionals with a focus on personalized physical therapy care and provide consultation to encourage aging in place, resource introductions, advocacy and travel companionship for seniors. We are a team dedicated to helping individuals and their families create memories while supporting our clients to lead the best quality of life for them.

We are a therapy and senior care team. Brittany Singleton is a Master's trained physical therapist. Jim Singleton is licensed physical therapy assistant. Brittany has been working with seniors for over 20 years in therapy services and hospice. Prior to that she received her Bachelor's in Exercise Science from Auburn University. She was a White House intern under the Clinton administration in the White House Athletic Center. Brittany has worked with primary care physician practices contracting initiatives, developed specialty programs in occupational medicine clinics. She has lead in-patient rehabilitation units in multiple hospitals, initiated therapy programs in 5 star senior living communities, and established an exceptional reputation in hospice care in Arizona and Nevada. Brittany is a certified senior advisor, an advanced clinical instructor through the APTA and was a certified wound specialist for over 10 years. Brittany established the first clinically integrated network (CIN) in the country for primary care in Dayton, Ohio. Jim is a graduate of the police academy and has his bachelor's degree in criminal justice from San Jose State University. A retired sheriff's deputy, Jim then set out to complete his PTA degree. He has had experience in the long term care setting, as well as, home health and currently, Jim has treated patients in the out-patient setting as a PTA for over 5 years. Together the Singleton's boast over 30 years of expertise advising and treating seniors.
We are a team of therapists and wellness specialists who will be able to assess your strength and conditioning needs to relieve pain, improve daily function, and get you back to doing what you love. We can also partner with loved ones and caregivers to establish a plan for the future. We work with communities to offer group fitness programs and develop a plan for people to live active lifestyles. We can be a resource as a private therapy solution post a traumatic event or a routine to stay active and healthy. We will be at the ready in the event that there is a health emergency or pre-planning to assure your wishes are met. We want to be an extension of your family to assure peace and comfort with wonderful memories and the absolute best care available to seniors.

1 hour evaluation and development of a plan of care to meet specific goals and/or lifestyle parameters.
1 hour treatment including stretching, exercise, massage, manual technique to reduce pain, increase mobility or increase healthy function. Maintenance physical therapy for progressive diagnosis and fall prevention utilizing the Otago Exercise Program. Each therapy session will conclude with 15 minutes of massage.
1 hour speech evaluation to complete an oral mechanism/cranial nerve assessment followed by a swallow evaluation to determine the patient's overall clinical status. Nutrition assessment as needed with consideration for alternative means of nutrition and hydration.
1 hour treatment including compensatory strategy training, strength training, education, alternative communication, and cognitive re-training such as reasoning, sequencing, problem solving, etc.
Including travel by car or plane; hotel overnight stays
Local trips < 30 miles for shopping, doctor appointments, errands, or to visit friends.
Light relaxing massage
Light muscular massage to assure the neck muscles are relaxed; can reduce swelling in the arms and hands. Not indicated for individuals with severe cardiovascular disease.
Light muscular massage to assure low back and leg muscles are relaxed and reduce swelling and edema. Not indicated for individuals with severe cardiovascular disease or open wounds.
Full body muscular massage with essential oils and cupping technique to reduce muscle soreness and spasm and promote better body awareness for balance and improved mobility. Not indicated for individuals with severe cardiovascular disease.
1 hour group fitness sessions either seated or standing or a combination of both. Exercises focused on fall prevention and rotation movements for improved safe function and core strength.
1 hour exercise session to include stretching, agility, posture correction, cardio workout, strengthening, and cool-down.
Consultation provided by zoom link to provide weekly check-ins or discuss with individual or families our offerings to gain resource knowledge.
1 hour consultation to discuss physical therapy education, aging in place services and education, resource identification in crisis situations, advocacy services, travel companionship details.
2 hour consultation can include physical therapy education, aging in place services and education, resource identification in crisis situations, advocacy services, travel companionship details, review and developing end of life notary documents
